1. Lack of slope compensation where duty cycle has exceeded 50%.
2. The switch mode inductor or transformer going up in to saturation for some of the switching cycle.
3. The magnetics not being too loosely wound, (that is, wound in a sloppy, careless way)
4. Oscillation on the output caused by incorrect SMPS loop compensation.
5. Dried capacitors.
